Satisfactory Evidence is NEEDED to PROVE your Identity

A CA State Issued Driver's License or State Issued Identification Card
ID card or driver’s license issued by another U.S. state
A US Military ID Card must include a Signature
A United States Passport
Employee ID card issued by an agency or office of the state of CA, or by an agency or office of a CA city, county, or city and county
ID card issued by a federally recognized tribal government
A Mexican or Canadian Driver's License
Passport issued by a foreign government
Consular identification document
Inmate ID card issued by the CA Dept. of Corrections and Rehabilitation (inmates in custody in prison only)
Any form of inmate ID issued by a sheriff’s department (inmates in custody in a local detention facility only)
Not Accepted: FBI ID cards are a federal agency and not acceptable

Any form of ID used to obtain a notary, must be current or issued within 5 years.

Physical Description,

Signature and

Serial Number
